HIPAA Compliance

Hi there! This is Curall Health’s HIPAA compliance/privacy practices notice. This notice describes how we use your personal health information or PHI, and how you can have access to this info as well. What is HIPAA?

HIPAA stands for the Health Insurance Portability and Accountability Act. It is a federal mandate that requires most businesses or entities to protect your healthcare data. To learn more follow this link: https://www.hhs.gov/hipaa/index.html.

How does Curall use and share your data?

To treat you: In order for us to dispense your medications, we need to communicate with your doctor, prescriber, nurse or other healthcare providers about you.

To get payment: Though we do not accept insurance, we still may need to use your information to obtain payment for our services.

For internal administrative purposes: To ensure accurate records and monitor the quality of service we provide you.

We may also use or disclose your health information without your consent for/to: Health and safety reasons, individuals or family members involved in your care, parents or legal guardians, organ and tissue donation requests, military purposes, lawsuit requests, workers compensation requests, law-enforcement requests, national security requests, coroner, medical-examiner or funeral director requests, correctional institutes, protective services for government officials, federal/state mandated reporting, and other business entities associating with Curall Health that are required by law to protect your personal health information (PHI).

Other uses: For any other uses of your PHI not covered by this notice, we will ask for your written authorization to disclose information. You have the right to revoke that permission in writing at any time.

What are your rights?

Get a copy of your medical records: In most cases, you have the right to attain a copy of your health records and information. All requests must be made in writing to care@curallhealth.com, a small fee may be charged for copies, shipping or handling.

Get a list of whom we shared your health info with: You can request a record of instances where we accessed/shared your health information for purposes other than treatment, payment and administrative uses.

Change your medical record if you think it’s wrong: If you believe there are inaccuracies or missing data in your Curall Health records, then you can request corrections in writing. We do reserve the right to deny your request after review.

Ask us to limit the information we share: You may request in writing that we limit the use or disclosure of your personal health information. Your request will be reviewed, but we are not legally bound to oblige.

Complain in writing: If you believe your privacy rights have been violated, you can send a written complaint to care@curallhealth.com. We will review your complaint and respond in a timely manner. You can also complain directly to the Department of Health and Human Services, here.
